Opening New Doors in the Fight Against DIPG

For decades, Dr. James Rutka has dedicated his career to improving the lives of children with brain tumors. As a pediatric neurosurgeon and Senior Scientist at The Hospital for Sick Children (SickKids) in Toronto, he has become one of the world’s leading experts in pediatric brain tumor research and treatment. His remarkable career has included more than 500 scientific publications, leadership of the Arthur and Sonia Labatt Brain Tumour Research Centre, and service as Editor-in-Chief of the prestigious Journal of Neurosurgery — the first and only Canadian to hold that role. In recognition of his contributions to medicine and research, he was named an Officer of the Order of Canada, one of the country’s highest civilian honors.

In 2025, Dr. Rutka received a ChadTough Defeat DIPG Game Changer Grant for his project, Enhancing Therapeutic Delivery in Diffuse Intrinsic Pontine Glioma. His research focuses on one of the greatest challenges in treating DIPG: getting medicine to the tumor. Because DIPG develops in the brainstem — a region that controls essential functions like breathing and heart rate — surgery is not an option. The tumor is also protected by the blood-brain barrier, a natural defense system that prevents many medications from reaching the brain. As a result, even promising cancer drugs often cannot reach DIPG tumors in sufficient amounts to be effective.

To overcome this challenge, Dr. Rutka and his team are using MR-guided focused ultrasound (MRgFUS), a technology that temporarily opens the blood-brain barrier and creates a window for treatment delivery. The approach has already advanced into a Health Canada-approved clinical trial, where researchers are testing whether focused ultrasound can improve delivery of chemotherapy to children with DIPG. At the same time, the team is studying how this technology may help activate the body’s immune system against the tumor.

Over the past year, the project has made significant progress. Researchers successfully optimized the focused ultrasound procedure and found early evidence that it changes the tumor’s immune environment, increasing the presence of immune cells in treated areas. These findings are helping the team better understand how focused ultrasound could work alongside emerging treatments such as immunotherapy.

As the research has progressed, the team has also expanded its focus to explore some of the most promising emerging approaches in DIPG treatment, including immunotherapy and CAR T-cell therapy. Two new collaborations are helping make that possible. Dr. Tim Phoenix brings expertise in DIPG models that preserve the blood-brain barrier, allowing researchers to more closely study what happens in the human disease. Dr. Nick Vitanza, a leader in CAR T-cell therapy for pediatric brain tumors, will work with the team to investigate whether focused ultrasound can help more CAR T cells reach DIPG tumors. Together, these collaborations expand the team’s ability to explore how focused ultrasound could be combined with powerful new therapies to improve their effectiveness.

Looking ahead, Dr. Rutka and his team will build on these findings by continuing to study how focused ultrasound affects the tumor’s immune environment and testing its ability to improve the delivery of immunotherapies. The ultimate goal is to identify the most promising combinations and lay the groundwork for future clinical trials — bringing more precise and effective treatment strategies closer to children with DIPG.

As Dr. Rutka and his team continue their work, every discovery brings the DIPG community one step closer to a future where children diagnosed with this devastating disease have more treatment options — and more hope.
